III. Documents Required
In case of the setup of a Sino-foreign joint venture or cooperative medical institution, the application together with the following documents shall be submitted to the health administrative department at the district and county levels where the medical institution is located:

(I) The application letter on the setup of medical institutions and all necessary documents for the approval of the setup of medical institutions (see notices on the approval of the setup of medical institutions for more information);

 (II) The project proposal signed by legal representatives of both parties of the joint venture or cooperative medical institutions and the feasibility study report for the setup of the joint venture or cooperative medical institutions;

 (III) The registration certifications (copy) of both parties of the joint venture or cooperative medical institutions, ID certification (copy) of legal representatives and the credit certification of the bank;

 (IV) Documents issued by the state-owned assets management authority for verifying the evaluation report of stated-owned assets to be invested.

The form should be filled with fountain pen, signing pen or be printed. The words used should be concise and legible and no blank column is left. The pattern and specification of the form downloaded from the Internet should not be altered. All documents required shall be submitted in sequence and be printed or copied with A4 paper affixing with the company seal on each cover of various documents.
